I don't know if this solution will work for you, Barry, but it solved the problems I was having.
In order to get the PDF document to look exactly as the MS Word document, you have to create a "custom page" in the Distiller Printer properties that matches the size of your MS Word page and print to that. So, when you print from Word, don't use the default paper size (8.5" x 11" or whatever you have there), just be sure to use the "custom" page you created.
I suspect the concept might be the same if you're scanning and then printing to PDF from that. Maybe create a portrait page that exactly specifies the dimensions you need, and which are specified in your Distiller job.option.
